Thomas Holmes Moore Jr.

November 6, 1945 — April 30, 2025

Thomas Moore, loved and respected family doctor in Rohnert Park, CA, for 33 years, passed away in San Diego on April 30th, following complications of a stroke.

Tom was born in 1945 in Laconia, NH, and grew up on the campus of the New Hampton School (NH) where his parents taught and from which he graduated in 1963. His youth was filled with the robust adventures that come with living in a rural New England community, a love of learning, and respect for place. His career path was inspired by the small-town family doctor who made house calls and took care of everyone from birth to old age. Tom’s caring and good-natured style of practice was the same. He visited his patients at home, and elderly patients were immensely grateful.

Tom graduated from Middlebury College in 1967, but his goal of a medical education was put on hold by the Vietnam War. He joined the Navy, which he credited with helping him grow up and for bringing him to San Diego where he met his wife, Tina, at a dance at the Officer’s Club. They married in 1971. Upon completion of his obligatory service, the Navy put Tom through medical school, and he graduated from Chicago College of Osteopathic Medicine in 1976, completed his family practice residency at Charleston Naval Hospital, and served as a Naval medical officer at Camp Pendleton in San Diego.

In 1981 Tom and Tina moved to Rohnert Park, CA, with their first two children, Jennifer and Noah, and two foster sons, Hung and The Tran, where they opened their combined medical and dental practices. Their third child, Polly, came soon thereafter. Dr. Tom Moore served the community of Rohnert Park from 1981 to 2014. His patients appreciated his steadiness, integrity, honesty, and sense of humor.

Retirement opened new doors for Tom. He found great pleasure and satisfaction right in his own backyard with a prolific garden and productive worm farm. He was a passionate beekeeper, inspired by the similar retirement gig of one of his favorite literary characters, Sherlock Holmes. Tom also sought to become a fluent Spanish speaker and made many friends through his online course. His most recent retirement obsession was pickleball, and the camaraderie he shared with his fellow Rohnert Park players, as well as new friends in San Diego, was precious to him.

Tom loved taking ski trips to Lake Tahoe with his children, who can attest to the fact that their Dad was a brilliant skier. He cherished many hiking adventures - Yosemite with his son, Noah; the John Muir Trail with his close friend Lee Eaton; and trips to Peru, the Canadian Rockies, and Newfoundland with Tina and family friends.

Tom and Tina relocated to San Diego in 2024 to be near their children and new grandson. Tom quickly picked up his old habits: he found a new pickleball family, planted a lovely vegetable garden, and Zoomed with his fellow Spanish students. He found pleasure in hiking the canyon trails near his new homeand was fascinated by the geology of the San Diego area.

Tom was grateful for his life and especially his family. He took pleasure in the simple fact that everyone got along and cared for each other, and he nurtured that connection whenever possible. He leaves behind a family who loved him deeply - his wife of 54 years, Tina; daughter, Jennifer (Harry Greer); son, Noah; daughter, Polly (Albert Shim); granddaughters, Chandler and Camryn Greer; grandson, Sonny Shim; as well as his siblings, Andy Moore (Suzi, deceased), Jamie Sheff (Paul), Rob (Laura), and Elibet Chase(George). Special thanks to the teams at Scripps La Jolla, Sharp at Kearny Mesa, Sharp Acute Rehab, and Sharp Home Hospice for giving Tom the same care and attention he provided to his patients throughout his medical career.

A service will be announced at a later date. Please donate, in Tom’s memory, to the good cause of your choice.
